This is an example widget to show how the Menu Sidebar Widget Area looks by default. You can add custom widgets from the widgets in the admin.

Download a high-resolution visual discomfort rating chart in PDF format at 300 DPI and print it on A4 or US Letter paper with 100% scaling to keep numeric values aligned with each expression.

Choose a layout that shows six to ten illustrated expressions arranged horizontally with clear numbers from 0 to 10 beneath each image. Use bold sans-serif text at 18–24 pt for visibility from 1–2 meters in clinics, classrooms, or home care settings.

For durability, print on 200–250 gsm cardstock and laminate with 80–125 micron pouches to protect against spills and frequent handling. If wall mounting is required, select an 11×17 inch format to improve readability across examination rooms.

Store digital files in both color and grayscale versions to match different printer capabilities, and keep a master copy unchanged to maintain sharp line quality for future reprints.

Print the visual discomfort rating chart at 300 DPI on 210×297 mm or 8.5×11 inch paper and set scaling to 100% to keep numbers precisely aligned with each illustrated expression.

Select a layout with 6, 8, or 10 graded expressions arranged horizontally, labeled from 0 to 10 beneath each image. Use high-contrast black line drawings for clarity in low-light hospital rooms, and choose sans-serif numerals at 20–24 pt for readability from two meters.

  • 0 = no discomfort, relaxed expression
  • 2–4 = mild soreness, slight frown
  • 6 = moderate distress, tightened eyes
  • 8 = strong discomfort, pronounced grimace
  • 10 = extreme suffering, tears illustrated

For pediatric use, laminate the chart with 100–125 micron film to withstand frequent cleaning with alcohol wipes. Rounded corners reduce risk of paper cuts in clinical settings.

Mount the sheet at eye level, approximately 120–140 cm from the floor in examination rooms, so children can point directly to the matching expression without assistance.

Store digital copies in both color and grayscale formats and label files by size, such as A4_color_300dpi.pdf, to simplify quick reprints during high patient volume periods.
